The Case Management Assistant supports the Case Management team working collaboratively in the Case Management Department and on the nursing units under the direction of the Director of Case Management. The Case Management Assistant interacts with patients, their families, clinical and ancillary staff, and community agency representatives to assist with the coordination of care and optimization of safe, timely, patient centered discharges.
Must have the ability to maintain strict confidentiality of patient information, medical records, etc.
High school diploma.
Associates degree desired.
Related and significant experience in the health care industry may be considered.
Must have the ability to work well under pressure and maintain composure with a positive, professional attitude to interact appropriately with hospital staff, patients, families, and community agency representatives.
Must be able to work independently, prioritize work and multitask.
Competent verbal and written communication skills.
