The Director of Social Services, under the direction and supervision of the SNU Administrator, is responsible for provision of social work functions for the Skilled Nursing Unit. This role includes psychosocial assessment, developing and implementing individualized social service care plans for patients and families served by the Agency to maximize patient potential for health and functioning. Coordinates social service activities within the department. Provides intervention and maintains competence in these areas. The Director of Social Services serves as a member of the interdisciplinary care team contributing to the understanding of the social, emotional, and economic factors that pertain to the patients care. The Director of Social Services supports the philosophy and objectives of the hospital, and all State and Federal guidelines.
Bachelor’s Degree from an accredited school of social work, licensed to practice in the state of Pennsylvania
The Social Worker must understand and demonstrate the social work roles of advocate, mediator, organizer, enabler, teacher, and facilitator as they are central to the provision of effective and comprehensive social work service. Ability to do oral presentations and teaching for groups and individuals. Demonstrates commitment to quality patient care. The BS must be licensed to practice social work in the State of Pennsylvania. Valid PA driver's license and dependable transportation is required. Current CPR certification is required.
