Assists the residents/registrants and families in matters which pertain to the adjustment and any other psychosocial or emotional problem which may affect his/her ability to benefit from receiving services at Guthrie Cortland and Nursing Rehabilitation Center. Acts as resident advocate. Carries out the admissions and discharge process as appropriate and necessary in the Nursing Facility. Understands and adheres to GCMC's Compliance Policy, Code of Conduct and Conflict of Interest policies. Keeps abreast of all pertinent federal, state and GCMC regulations, laws, and policies as they presently exist and as they are changed or modified. Routinely assesses the resident and ADHC registrants for psychosocial needs and develops plans and interventions as needed to address such needs.Requirements
Bachelor of Science in Social Work or related Bachelors Degree with appropriate experience at minimum, Masters in Social Work for LMSW and LCSW positions.
