To provide patients and their families with psychosocial interventions and discharge planning. The Social Worker is responsible for addressing complex bio-psychosocial needs of the patient. The scope of practice includes, but is not limited to comprehensive assessment of those needs, diagnosis, advocacy, and resource identification and linkage. Adjustment to illness issues, mental health, healthcare decision making, substance abuse, child and elder abuse and neglect, domestic violence, and concrete services are addressed within their setting, and as the patient flows through the system.Requirements
Bachelor of Science in Social Work or related Bachelors Degree with appropriate experience at minimum, Masters in Social Work for LMSW and LCSW positions.
