Guthrie Medical Group-Albro
Responsible for health promotion and illness prevention of patient populations through nursing assessment and patient education. Performs the Medicare Annual Wellness Visit under the direct supervision of a physician (physician on site) for beneficiaries in order to identify a personalized and individualized prevention plan, complete with objectives and goals for patients as appropriate. Collaborates with physicians for further counseling and treatment as indicated. Maintains knowledge of quality measures set forth by federal programs and ensures adequate documentation in the EHR. Will work collaboratively with clinical and financial and operational leadership.
A nursing diploma (RN) and state licensure is required.
The ambulatory wellness nurse will need to demonstrate clinical expertise in identifying patient risk factors and developing a plan for illness prevention and wellness promotion. The nurse will need the ability to communicate effectively both verbally and in writing to patients of varying ages, developmental stages, and education levels. He or she will need to facilitate the coordination of patients care with physicians and mid-levels, and provide thorough education to the patient and patient’s family regarding disease process, illness prevention, risk factor modification, and health promotion. Ability to comprehend quality measures and ensure adequate documentation in the EHR is required. Operates PC and other software as required to perform job duties. Maintains effective interpersonal relations.
