Shift

PT Days - 8 hours, with benefits

32 hours/week, Mon, Tues, Thurs, Fri

License

RN , PA and NY dual license preferred

Main Function

Performs anticoagulation studies in a highly structured environment by collecting specimens, taking vital signs, and patient history. Assessment of anticoagulation report and medication adjustment based on standardardized protocols and algorithms. Collaboration with medical director on new patient referrals, risk assessment scoring, and bridging therapy requirements. Documents, reviews and enters all patient data in the electronic medical record including medication review. Provides ongoing patient education and counseling. Generates reports to providers. Assists patients with medication management.

Job Duties and Responsibilities:

  • Performs liaison duties between patients, referring providers, hospital staff and management.
  • Performs laboratory procedures
  • Performs daily quality measures
  • Maintains daily control and testing logs
  • Chart review of all newly referred patients with overview of medical condition and plan review with medical director.
  • Patient history, vital signs, review of current medication, review of laboratory results with patient, review of medication recommendation with patient
  • Chart review for drug to drug interactions with warfarin.
  • Working knowledge and understanding of risk and interaction with chemotherapy drugs and appropriate anticoagulation monitoring
  • Adjustment of medication by standard protocol and algorithms based on history and patient evaluation
  • Chart documentation, data entry into electronic medical record.
  • Responsible for risk score analysis and review with medical director
  • Provide pre-procedure bridging therapy assessment.
  • Will instruct patient on giving self injection and coordination of care with proceduralist
  • Ongoing patient education on warfarin therapy management
  • Warfarin therapy compliance review with patient
  • Assists in the departmental operations according to approved policy and procedure
  • Additional duties and responsibilities as required by the administrative staff.

Education

Graduate of approved school of nursing

Experience

Experience in a clinical setting preferred. Knowledge of medical terminology required. Should possess working knowledge of technical policies and procedures relevant to performing tests in a regulated environment. Should be proficient in clerical duties, organizing workload and computer skills. Excellent telephone etiquette and communication skills required. Job requires proficiency in mathematics and the ability to convert milligrams per week into number of pills per day and strong pharmaceutical knowledge.
