The Pulmonary Disease Navigator is a patient educator and advocate, care coordinator, system navigator, and community ambassador on a mission to improve the experience for the individual affected with a diagnosis of pulmonary disease by being a support system for the patient and his/her family. The Pulmonary Disease Navigator will assist patients with all types of pulmonary disease with an emphasis on patients with chronic obstructive pulmonary disease. Works collaboratively and maintains active communication with physicians, respiratory therapists, nursing, and other members of the multi-disciplinary care team to effect timely, appropriate patient management for COPD patients. Maintains close communication and collaboration with physician advisors. Optimize and improve patient, physician, organizational, and community clinical quality, service quality and quality outcomes by providing consistent and timely care coordination, while reducing the healthcare costs.
The Pulmonary Disease Navigator must be a graduate of a School of Respiratory Therapy approved by CoARC and currently a licensed Respiratory Care Practitioner in Pennsylvania. Credentialed as a COPD educator via the American Association of Respiratory Care (AARC) course within 1 year of hire. Smoking Cessation counselor course must be completed within 1 year of hire. BLS required.
The Pulmonary Disease Navigator must have a minimum of five years experience in a clinical setting and demonstrated abilities in establishing positive relationships with patients, families, hospital and clinic staff; and excellent case management, collaborative practice, organizational, communication and teaching skills. Preferred experience in the development, coordination and performance of educational and/or support programs and leadership.
