The Patient Centered Medical Home (PCMH) Project Manager is responsible for overall project management of the PCMH project work in primary care. National Center for Quality Assurance (NCQA) compliance with annual reporting measures in all primary care locations. This includes on-site review at all primary care locations. The PCMH Project Manager/Analyst will be responsible for the review and development of all required workflows in Epic to comply with the latest NCQA Patient Centered Medical Home Standards. This includes review of criteria, development at the practice level and surveillance for compliance. The PCMH PM/Analyst will utilize all NCQA on-line tools for application, and continued survey readiness in assisting the practices success with recognition process. Collaborates closely with all disciplines in Population Health Development, to include, but not limited to: Quality, Care Coordination and Case Management, Epic, and organizational leadership. Acts as a liaison between physicians, administration and clinical leadership.
Bachelor’s degree required, Master’s preferred. Project Management Certification (PMP) desired. Registered Nurse desired.
2 years of experience with the PCMH model of care
Proficient in the use of Epic with at least 2 years of experience, Epic System Certification desired within the first year
